Closes #670 (audit/documentation portion; controller retains final disposition).

What

Adds docs/incidents/670-direct-master-commit-2fa97c26.md: a durable retroactive audit of the bare direct-to-master commit 2fa97c26fbda555a1a83930ca5fdcea9d8e47b50 (fix(mcp): load dotenv relative to project root), discovered during the PR #654 post-merge audit.

Acceptance-criteria coverage

AC1 — Verified 2fa97c26 is an ancestor of prgs/master (git merge-base --is-ancestor).
AC2 — Confirmed no PR wrapper and no review record: single-parent non-merge commit sitting on first-parent master between the #629 merge (5ab5fe85) and the #654 merge (ec903b0d); controller audit at issue-create also found none.
AC3 — Diff confirmed: gitea_auth.py | 5 +++-- (+3/−2), single parent 5ab5fe85.
AC4 — Documents why no immediate revert: the dotenv fix is intentional and required for runtime .env resolution; revert would re-introduce a real bug to remove a provenance defect. Controller keeps final disposition; a separate revert issue is the path if later judged unsafe.
AC5 — Links the landed hardening: issue #671 (closed), commit 5933d87647656643a67a50331c4c7b06ea751dad, shipping gitea_record_stable_branch_push_attempt + reconciler-only gitea_audit_stable_branch_contamination.
AC6 — Durable audit trail that PR #654 was not the source: 2fa97c26 is the first parent of merge ec903b0d and predates it; #654 merged reviewed #603 content only.

Non-actions (unchanged)

No revert, no force-push/history rewrite, no master mutation.

Validation
